I have insurance, so why must I pay to see the doctor?
Unless you have medicare and medical, or medicare and another insurance plan, all plans have a copay that you are responsible of paying no matter what provider or doctor you visit. Depending on your plan you may also be responsible for a deductible or "out of pocket" amount in addition to your coinsurance or copay. If you want to know what your copay, coinsurance, and "out of pocket"/deductible amounts are, the front desk can help you or you can call your insurance company for a full summary of benefits.
What is a deductible?
A deductible is the amount that the patient/subscriber pays every year ( a set amount in your insurance contract) for your insurance to be eligible to pay fo the remaining treament/visits of the year.
What is an "out of pocket?"
The "out of pocket" amount is another set amount in your insurance contract that you pay before your insurance company picks up all of your expenses at 100%. This amount is separate from the deductible and some insurance plans may not have one.
What kind of insurance do you accept?
We accept all PPO plans, medicare, and medi-cal only if with medicare.
Do you accept worker's comp?
Yes, if there is proper authorization for the visit and if the visit is medically necessary.
Do you accept liens?
No, sorry we do not.
How do I obtain copies of medical record?
Due to recent privacy laws you must sign a release of records form, which is available at our office any time.
